The Town Clerk's Office provides FREE Town of Hempstead Senior I.D. Cards, complete with a resident's personal data, physicians' contact information and emergency phone numbers. Allergies and other vital information can also be printed on these wallet-sized cards. Seniors will also get an 8 1/2" X 8" form that can be placed on their refrigerator doors.
These Senior I.D. Cards and forms may be useful in the event of an emergency, as medical technicians can quickly gather pertinent information about a senior and contact his/her doctors and family members.
